Sometimes a young person’s success depends on more than what happens in a classroom or Club program. Families facing housing instability, food insecurity, transportation challenges, family crises, or other barriers often need additional support. Our Case Management team works alongside youth and families to help them access resources, overcome challenges, and build a path toward long-term stability.

Our Case Management team meets families where they are, providing individualized guidance, advocacy, and connections to resources that address immediate needs while supporting healthy, sustainable outcomes. How We Support Youth and Families

Our Case Management services may include:

• Advocacy and navigation of housing, healthcare, education, and other essential systems
• Food assistance, clothing, shoes, and support with other basic needs
• Emergency assistance and connections to community resources
• Crisis intervention, emotional support, and counseling referrals
• Goal setting, life skills development, and individualized family support plans
• Diversion and restorative justice services focused on accountability and growth
• Family reunification monitoring and ongoing support
• Referrals and connections to public and private support services
